What you'll need
Here's what helps your application go smoothly. You don't need all of it to start — you can save and come back as you gather things.
About you and the victim
- Names, dates of birth, address, and phone number
- If you're applying for someone else (for example, a child), your relationship to them
About the crime
- The date and where it happened
- The police report number, if you have one (it's okay if you don't)
Costs and losses
- Bills or receipts (medical, counseling, funeral, etc.)
- If you missed work: your employer's name and the dates you missed
- Insurance information, if any costs were covered
